Please forward by mail, email or facsimile to the attention of Partner Relations the following information:• Your Name• Old Social Security Number or Tax ID Number for verification• Correct Social Security Number or Tax ID Number• Daytime telephone number including area code• Email address • Or completed Form W-9 How do I report a change of ownership as a result of death?• For all changes of ownership as a result of death, please send written notification via mail, email or facsimile to the attention of Partner Relations with the following information:• Deceased owner’s name• Contact person’s name, daytime telephone number, address and email address• Copy of the decedent’s death certificate• Copy of the personal representative/executor/executrix letters• Estates Federal EINFurther instructions or requests for information will then be sent to you.All ownership changes require the consent of the general partner and will be effective as of December 31st of the year the general partner consents.
